Innovador procedimiento quirúrgico de médicos de Almenara salva la vida de paciente con tumor en el oído.

La parálisis facial que padecía tenía ese origen: tumoración en el oído izquierdo, sin embargo este equipo de especialistas del hospital Almenara asumió el reto de salvar su vida con la aplicación de un procedimiento quirúrgico nunca antes practicado en el país.

Por primera vez en el Perú y en el Hospital Guillermo Almenara del Seguro Social (EsSalud) fue realizada una cirugía por vía de abordaje del oído para extraer un tumor que causó parálisis facial en este ayacuchano de 59 años.

Alfredo Sánchez Kamada, empezó con una infección en el oído que le provocaba problemas de equilibrio. En abril de este año, el señor Sánchez llego por emergencia al Hospital Almenara debido que tuvo una parálisis facial del lado izquierdo y así fue hospitalizado inmediatamente.

El doctor Omar González Suazo, jefe del Servicio de Otorrinolaringología del Hospital Almenara explica: “se le realizaron múltiples exámenes de ayuda diagnóstica, exámenes tomográficos y de angioresonancia del oído. También se le realizo una exploración por vía endoscópica, con la cual se ubicó la tumoración, en el oído interno. Esta comprometía el séptimo y octavo par craneal, nervio auditivo y nervio facial. Se realizó una investigación exhaustiva y un plan quirúrgico”.

Desde ese momento, las reuniones clínicas del servicio fueron constantes pues debían evaluar la manera de abordaje quirúrgico que evite cualquier compromiso o lesión que ponga en riesgo su vida.

“Al determinarse el compromiso del oído interno, se vio la posibilidad de hacer un abordaje por vía endoscópica nasal para llegar a la zona de la tumoración, lo cual es más común o hacer un abordaje a través del oído, lo que podría tener menos complicaciones posteriores a la cirugía”, señalo el Dr. González.

Decidieron realizar la operación por el oído y esta fue un éxito, el paciente no tuvo que estar postrado, su recuperación fue mucho más rápida y recobro lucidez 2 horas después de la cirugía. Es la primera vez en el Perú que se realiza esta cirugía con vía de abordaje por el oído. Comúnmente es realizada por los neurocirujanos desde la base de cráneo o la nariz.

“Esta cirugía se realizó con dos tipos de técnicas quirúrgicas, con un microscopio y con ayuda de un endoscopio. Son técnicas que las estamos empleando muy bien en el servicio, al igual que se realiza en Europa o en Estados Unidos”

Gracias a este novedoso procedimiento se evitó el compromiso de vasos sanguíneos importantes como el golfo de la yugular o la carótida interna.

“La evolución del paciente ha sido muy buena y rápida, todavía presenta parálisis facial, pero ha sido muy favorable la extirpación del tumor en forma completa, evitar alguna posibilidad de infección y lo más importante que es dar tranquilidad a la familia”

Las infecciones crónicas pueden tener complicaciones como una perforación del tímpano, parálisis facial, cuadros vertiginosos o producir tumoraciones en el oído interno, como el caso de Alfredo Sánchez.

From that moment, the clinical meetings of the service were constant because they had to evaluate the way of surgical approach that avoids any commitment or injury that puts their life at risk.

“When determining the involvement of the inner ear, it was possible to make an endoscopic nasal approach to reach the area of ​​the tumor, which is more common, or to make an approach through the ear, which could have fewer complications after surgery ”, stated Dr. González.

They decided to perform the operation through the ear and this was a success, the patient did not have to be prostrate, his recovery was much faster and he regained clarity 2 hours after surgery. It is the first time in Peru that this surgery has been performed with an approach through the ear. It is commonly performed by neurosurgeons from the base of the skull or the nose.

“This surgery was performed with two types of surgical techniques, with a microscope and with the help of an endoscope. They are techniques that we are using very well in the service, as is done in Europe or the United States “

Thanks to this novel procedure, the compromise of important blood vessels such as the gulf of the jugular or the internal carotid was avoided.

“The evolution of the patient has been very good and fast, he still has facial paralysis, but the complete removal of the tumor has been very favorable, avoiding any possibility of infection and the most important thing is to give peace of mind to the family”

Chronic infections can have complications such as a perforation of the eardrum, facial paralysis, vertiginous pictures or produce tumors in the inner ear, as in the case of Alfredo Sánchez.
