Nutrición balanceada y personalizada evita muertes en pacientes hospitalizados en UCI-Covid.

Según data el área UCI-Covid, el 95% de los pacientes hospitalizados ingresó con riesgo nutricional.

El licenciado Brian Mariños, encargado de las Unidades de Cuidados Intensivos Covid del Hospital Nacional Alberto Sabogal, indicó que el riesgo nutricional en pacientes de área crítica, se ha incrementado considerablemente, de 70 a 95% en pandemia, lo que obliga a empezar la estrategia nutricional lo más pronto posible. El especialista primero diagnostica, luego diseña el plan de intervención y monitoreo, finalmente vierte la información a la historia clínica para evaluar el proceso.

“El riesgo nutricional se relaciona mucho con situaciones desfavorables como mayor estancia hospitalaria, mayores días en ventilación mecánica y mayor riesgo de mortalidad o complicaciones infecciosas”, señaló el especialista de EsSalud.

Sin embargo, la desnutrición no siempre está asociada a la baja de peso. Los pacientes con obesidad también pueden ingresar con riesgo de desnutrición, eso se debe a que no estarían consumiendo alimento acorde a sus requerimientos. Una persona con sobrepeso podría tener incluso mayor riesgo de mortalidad por desnutrición, que una persona delgada.

NUTRICIÓN EN CUIDADOS INTENSIVOS
Durante el paso del paciente por la Unidad de Cuidados Intensivos, pueden existir variables que generen desgaste de su estado nutricional. Por ello los especialistas hacen seguimiento diario de lo que consume y lo que debe consumir.

De acuerdo al Protocolo de Atención Nutricional, primero se valora la talla y peso del paciente. En las primeras 48 horas de hospitalización UCI, se prescribe un consumo de 20 calorías y 0.8 gramos de proteínas por kilo de peso ideal.

Pasadas las 48 horas, que el paciente ya tiende a la estabilidad hemodinámica y metabólica, se indican 25 calorías y 1.3gr de proteínas al día. Tras la primera semana de hospitalización, esta dieta se incrementa hasta 30 calorías y 1.5gr de proteínas por kilo de peso ideal.

POST EXTURBACIÓN
Al ser extubado se inicia la alimentación del paciente por vía oral, sin quitarle el acceso a la sonda. Esta será retirada cuando ya pueda consumir más del 60% de su requerimiento por vía oral. De igual manera se le suplementa con fórmulas enterales que el hospital provee, para así llegar a la suficiencia de macro y micronutrientes que un cuerpo requiere.

Cuando el paciente sale de alta, el equipo de nutricionistas personaliza el plan de alimentación de acuerdo a su estado de salud, pero también en virtud a la presencia de otras condiciones o enfermedades como diabetes mellitus, hipertensión o enfermedad, renal que podrían determinar variación en la dieta.

Una vez que el paciente inicia la fisioterapia la dieta es replanteada, ya que para entonces necesitará un mayor aporte de calorías y proteínas. Otro factor que eleva el aporte proteico es la presencia de úlceras por presión o escaras debido a ciclos de inmovilidad. En este caso la proteína servirá para generar el cierre de las heridas y evitar el riesgo de infección.

Para los especialistas de UCI-Covid el seguimiento médico por Tele-consulta es de suma importancia, ya que permite para variar las estrategias de acuerdo a la evolución de paciente. En casos extremos, el desgaste nutricional por Covid-19 puede estar reflejarse hasta tres años después de haberse contagiado.

Durante la pandemia, las Unidades de Cuidados Intensivos Covid del Hospital Nacional Alberto Sabogal viene atendiendo a más de 580 pacientes por día, asimismo se han elaborado más de 16 mil dietas por mes.

Si bien es cierto que el 95% de los pacientes llega a UCI-Covid con riesgo nutricional, esta estrategia ha permitido que, actualmente se reduzca el riesgo de mortalidad al 30% con buena supervivencia en UCI.

NUTRITION IN INTENSIVE CARE
During the patient’s passage through the Intensive Care Unit, there may be variables that cause deterioration of their nutritional status. That is why the specialists make daily monitoring of what you consume and what you should consume.

According to the Nutritional Care Protocol, the height and weight of the patient are first assessed. In the first 48 hours of ICU hospitalization, a consumption of 20 calories and 0.8 grams of protein per kilo of ideal weight is prescribed.

Once the patient begins physiotherapy, the diet is rethought, since by then he will need a greater intake of calories and protein. Another factor that increases protein intake is the presence of pressure ulcers or bedsores due to cycles of immobility. In this case, the protein will serve to generate the closure of the wounds and avoid the risk of infection.

For the UCI-Covid specialists, the medical follow-up by Tele-consultation is of the utmost importance, since it allows to vary the strategies according to the patient’s evolution. In extreme cases, the nutritional attrition by Covid-19 can be reflected up to three years after being infected.
